Differential Diagnosis
Maria
Maria’s significant levels of IgE to Phl p 1 and Phl p 5b indicate primary grass pollen sensitization. SIT treatment for Timothy grass is suggested since the component specific IgE antibody pattern indicates Timothy as the primary sensitizer. The profilin (Phl p 12 and Bet v 2) response explains the positive birch extract result since profilin also is found in plant food, tree, and weed pollen.1

Richard
Richard does not show sensitization to the grass-specific components. Together with the significant IgE response to the cross-reacting components Phl p 7 (polcalcin) and Phl p 12 (profilin), this indicates that the pollen sensitization is not originating from grass pollen.1,2 The polcalcin (Phl p 7 and Bet v 4) and profilin (Phl p 12 and Bet v 2) responses explain the positive birch extract result.1,2
REFINED DIAGNOSIS
- No primary grass pollen allergy.
HEALTHCARE PROVIDER MANAGEMENT PLAN
- Richard is advised by his healthcare provider to undergo further testing to understand what pollen can explain his sensitization pattern.
- Further investigation identifies mugwort as a primary sensitization source for Richard.
